How do I receive physical therapy?

Physical therapy can be provided with or without a referral in the state of Ohio. Some insurance companies may still require a referral, so we advise checking first. If you have ever been referred for physical therapy it is important to know that you have the option of going to the physical therapist suggested by your primary care provider OR choosing another physical therapist who you already have a relationship with or is more convenient for you in some way. While you may need to factor other things like in-network vs out-of-network and estimated cost per visit, at the end of the day your physical therapist is your choice! We would love to be your physical therapy provider of choice for all your movement-related dysfunctions.

How do I schedule an appointment?

Mt Lookout Chiropractic is located conveniently off of Delta Ave between Columbia Parkway and Mt Lookout Square. We have hours convenient to almost all patients. Call us at (513) 321-8484 to schedule your appointment.
If you have plan to use your insurance plan, please have your prescription and insurance card available when you call. This will allow us to verify your insurance benefits and provide you with an estimate of coverage.

What do I wear?

Dress comfortably. At the initial visit we recommend wearing clothes that expose, or allow for exposure of the involved body parts as much as possible. You are welcome to bring a change of clothes with you and change at our facility if needed. Athletic clothes and shoes are a good idea.

What can I expect my first visit?

The physical therapist will complete your evaluation, which typically consists of getting a thorough past medical history and testing your functional movement, flexibility and strength; and initiate treatment to help eliminate pain and establish a unique and specific plan of care.

Will the physical therapist communicate with my health care provider?

Mt Lookout Chiropractic and Sports Injury Center recognizes that open, honest communication among the members of your health care team (you, your therapist, your health care provider, etc.) is essential to making your rehabilitation experience a success. At your request, the physical therapist will send your health care provider a complete written report that includes the findings of the initial evaluation, the assessment of the injury, and a detailed description of the treatment plan.

Additionally, the therapist will not hesitate to contact your provider by phone if he/she has a specific concern or question. At the end of your treatment, the physical therapist will send a final report to your health care provider with information regarding your status at the time of discharge from physical therapy.

What type of follow-up is offered?

At Mt Lookout Chiropractic, our well-trained, professional staff emphasizes the importance of continuing your treatment with a home exercise program. Your therapist will design an at-home program for you and provide detailed instructions. This will allow you to continue the progress you made while in therapy and avoid further injury. We also offer a maintenance program upon discharge to ensure you continue to make progress and reduce the likelihood and/or severity of future exacerbations. We are partners in your recovery; so even after discharge, if you have questions, call us and we will be more than happy to help you with any questions or needs you may have.
